Job summary

Honor Community Health is seeking a Community Health Worker I for Homeless Health Services in Michigan. This in-person role supports individuals experiencing homelessness, coordinating medical, behavioral health, and housing resources.

The CHW engages with clients using trauma-informed, recovery-oriented approaches and partners with a multidisciplinary team. The position emphasizes navigation of community resources, client advocacy, and building trust in health centers, shelters, and other

Qualifications

• High school diploma or equivalent is required.
• One year of experience in healthcare, behavioral health, homeless services, or related field preferred.
• Current MiCHWA CHW certification or ability to obtain within 12 months of hire.
• Must obtain Medicaid reimbursement credentials (MI Medicaid CHW Registry and CHAMPS).
• Strong computer skills in Epic and Microsoft Office.

Responsibilities

• Serve as liaison, advocate, and resource navigator for individuals experiencing homelessness or housing instability.
• Collaborate with an interdisciplinary team to improve access to medical and behavioral health services.
• Assist clients in navigating housing resources, public benefits, and community services.
• Engage clients in trauma-informed, person-centered, recovery-oriented care across multiple settings.
