Job summary

Asura is seeking a Homecare Services Coordinator (Advisor) in Minnesota to develop and manage a service delivery plan under the CFSS program. You will work with participants, agencies, and DHS to ensure the plan is carried out and updated as needed.

The role requires strong communication, organization, and problem-solving skills, with an ability to navigate regulations, budgets, and service plans to help participants live more independently.

Qualifications

• Bachelor's degree in Human Services, Social Work, Psychology, Healthcare, Rehabilitation, or a related field is required.
• 1 year of full-time experience providing direct services to individuals with disabilities or older adults is acceptable.
• Personal experience coordinating your own long-term support services is also acceptable.

Responsibilities

• Develop and manage a service delivery plan with participants under the CFSS program.
• Coordinate with agencies, lead agency providers, and DHS to monitor plan progress.
