Qualifications

• Board certified with a valid NP/PA credential in Georgia.
• Strong clinical skills to assess, diagnose and manage care.
• 1–2 years in primary care with geriatric patients; hospice a plus.
• Ability to work independently and solve problems.

Responsibilities

• Provide high-quality primary and palliative care in the Griffin area.
• Perform exams, update histories, and plan patient care.
• Communicate with office staff and clinical operations systems.
• Deliver patient-centered care to a panel of homebound patients.
• Develop impressions, diagnoses, and treatment plans.
• Consult with patients, families, and caregivers during visits.
• Order diagnostic tests and prescribe medications as needed.
