Job summary

Ennoble Care seeks an experienced Advanced Provider (NP or PA) to deliver primary and palliative care to homebound patients in the Kansas City region (KS and MO).

You will perform exams, develop care plans, and coordinate with a physician and our in-house team to ensure patient-centered care for chronic conditions.

Board-certified in KS/MO with 1–2 years in primary care; hospice experience preferred; strong clinical and communication skills are essential.

Qualifications

• Board certified with valid NP or PA license in Kansas and Missouri.
• Strong clinical skills to assess, diagnose, and treat.
• Positive demeanor and people skills; able to work independently.
• 1–2 years of primary care experience with geriatric patients; hospice experience preferred.

Responsibilities

• Provide high quality primary and palliative care to homebound patients in KS/MO.
• Perform physical exams, review histories, and create care plans.
• Coordinate with physician and clinical operations; order tests as needed.
• Prescribe medications and necessary interventions; maintain patient relationships.
