Job summary

BCFS Health and Human Services in Tyler, TX is seeking a Home Visitor/Parent Educator to deliver quality services to families under the Texas Home Visiting Program, using SafeCare-based practices.

You will conduct intake and assessments, provide case management, education, referrals, and coordinate with the Program Director; must maintain documentation in PEIRS, Salesforce, and SafeCare.

Qualifications

• High school diploma or equivalent required; bachelor's preferred.
• Minimum 5 years of related field experience.
• Bilingual Spanish preferred.

Responsibilities

• Outreach and recruitment of participants for the Texas Home Visiting Program.
• Establish and maintain a trusting and professional relationship with families through regular contact.
• Utilize the Safecare curriculum with caregivers and report to the Program Director on recordings.
• Administer intake, assessments, and screening tools to clients and family members.
• Use intake results to set goals and activities with parents.
• Provide intensive case management services to families.
• Deliver parent and health education, referrals, and transportation assistance.
• Conduct follow-ups for all families.
• Create and set up events on monthly calendar.
• Record services in PEIRS (DFPS), Salesforce, and BCFS software.
• Meet with supervisor bi-weekly to review caseload status.
• Meet all grant deliverables and attend required trainings.
• Travel as needed and work with a multidisciplinary team.
