Job summary

Healing Partners seeks a Nurse Practitioner for home visits to deliver advanced wound care for patients transitioning from SNFs to home. You will perform in-home assessments, develop treatment plans, and coordinate care with primary care, specialists, and home health teams.

Travel within the assigned market is required, with a Monday–Friday schedule. This full-time role offers medical/dental/vision insurance, 401(k), and bonus opportunities for high-quality care.

Qualifications

Master's level NP with valid APRN license and DEA registration.
Experience in outpatient or home-based wound care preferred.
Strong clinical and communication skills for home settings.

Responsibilities

Perform in-home wound assessments and evaluate healing progress.
Develop and manage individualized wound care plans.
Coordinate care with primary care, specialists, and home health.
Ensure safe transitions from SNFs to home care.
Document within EMR and ensure regulatory compliance.
