Job summary

The Visiting Nurse Association of the Treasure Coast is seeking a Patient Care Manager - Home Health (RN) to lead a clinical team and ensure high-quality home-based care in the Vero Beach area. This role blends hands-on nursing leadership with operational oversight to ensure Care Plans translate into reliable patient outcomes.

As a clinical leader, you will coach RNs and field staff, monitor acuity and adherence to OASIS and 485/Plan of Care processes, and collaborate with scheduling to balance

Qualifications

• Current, unrestricted Florida Registered Nurse license.
• Home Health experience preferred and supervisory skills.
• Knowledge of OASIS, 485/Plan of Care, and care coordination.

Responsibilities

• Provide daily clinical leadership and coaching to the home health team.
• Oversee caseloads, acuity, clinical risks, and continuity of care.
• Review Start of Care, Recertification, discharge, and episode workflows.
• Monitor 485/Plan of Care, interventions, and changes in condition.
• Partner with scheduling to balance caseload, geography, and capacity.
• Support QA, regulatory readiness, and performance improvement initiatives.
