You will develop personalized treatment plans, collaborate with a healthcare team, and help patients improve mobility and quality of life in their own homes. The role requires current CA PTA license, fingerprint clearance, CPR/ACLS, immunizations, and a valid driver's license.

Qualifications

• Current Physical Therapist Assistant license in California.
• CA Fingerprint Clearance Card.
• Up-to-date BLS (CPR) / ACLS certification.
• Recent TB test and Flu shot within the past year.
• Immunization records including Covid-19, Hep B, MMR, TDaP, Varicella.
• Valid Driver's License with auto-insurance.

Responsibilities

• Deliver skilled physical therapy interventions to improve mobility and function.
• Create and execute individualized treatment plans for each patient.
• Collaborate with a multidisciplinary healthcare team to coordinate care.
