ABL Health Care is seeking a dedicated Physical Therapist Assistant (PTA) for Home Health in San Jose. The role involves evaluating patients, developing personalized treatment plans, and supporting mobility and quality of life in the home setting.

The ideal candidate holds a California PTA license, has at least one year of home health experience, a valid California driver’s license, and is willing to travel within a 15-25 mile radius. Join our collaborative interdisciplinary team.

Qualifications

• Current Physical Therapist Assistant (PTA) License to practice in California.
• Experience in Home Health care – at least 1 year.
• Current California driver’s license.
• Able to travel within a 15‑25 mile radius on a regular basis.

Responsibilities

• Provide physical therapy services within the scope of care, following the patient’s individualized plan, in coordination with the Interdisciplinary Team.
• Administer treatments, monitor patient conditions, and report signs/symptoms indicating a change in patient condition to physician, case manager, and/or director of nursing as needed.
• Maintain accurate clinical records and submit documentation according to company policy using a company‑provided device.
• Comply with all company policies and procedures to ensure high standards of care.
