Job Juncture is seeking a passionate Physical Therapist Assistant to provide high-quality home health care in San Francisco, CA. The role focuses on delivering personalized interventions, advancing mobility, and enhancing patients' daily functioning in their homes.

The successful candidate will hold a California PTA license, maintain current immunizations, TB/flu screening, and a valid driver’s license. This position offers hourly compensation between $40 and $50, with relocation assistance

Qualifications

• Current Physical Therapist Assistant license in the State of California.
• Recent Negative TB test result and Flu Shot within the past year.
• Comprehensive Immunization/Vaccination Records, including Covid19, Hep B, MMR, TDaP, and Varicella.
• Valid Driver's License along with auto-insurance.

Responsibilities

• Delivering skilled physical therapy interventions, including exercises and stretches, aimed at enhancing patients' mobility and function.
• Creating and executing individualized treatment plans tailored to each patient's specific needs and goals.
• Collaborating with a multidisciplinary team of healthcare professionals to ensure holistic and coordinated patient care.
