Parker At Your Door, a medical house calls program by Parker Jewish Institute for Health Care and Rehabilitation, seeks a Primary Care Physician (MD/DO) to deliver quality primary care to patients at home in the greater New York Metro region.

This role offers Full Time, Part Time, and Per Visit opportunities; requires NY licensure, strong communication, geriatrics/home care experience, and collaboration with an interdisciplinary team. Flexible scheduling offered.

Qualifications

• NY licensure and current registration to practice medicine in New York State.
• Strong communication and follow-up skills.
• Knowledge of Microsoft Office including Excel and data entry.
• Prior experience with geriatric population and home care preferred.
• Knowledge of medical documentation and record keeping.

Responsibilities

• Examine patients and order appropriate diagnostic workups.
• Assess changes in condition in a timely manner.
• Maintain complete and accurate medical records.
• Consult with interdisciplinary team to create care plans.
• Educate patients on disease processes, medications, and adherence.
