Job summary
Parker Jewish Institute for Health Care and Rehabilitation, located on the Queens-Nassau County border in New Hyde Park, NY, is seeking a Primary Care NP to provide in-home primary care to our patients in the greater New York Metro region.
This position offers Full Time, Part Time and Per Visit opportunities, with responsibilities including comprehensive patient evaluations, medical management, and collaboration with an interdisciplinary team.
Qualifications
- Licensure and current registration to practice as a NP in New York State
- Knowledge of Microsoft including Excel, data entry, documentation and report generation
- Prior experience with geriatric population and home care strongly preferred
- Knowledge of medical documentation
Responsibilities
- Examine patients and order appropriate diagnostic workups
- Assess members with acute change in condition in a timely manner
- Maintains complete and accurate medical records
- Consults with other members of the interdisciplinary team to create appropriate and viable care plan
- Educate patients in topics such as disease process, medication and compliance
- Medical management of patients
