Job summary

Optum Home & Community Care is expanding its HouseCalls program to deliver in‑home preventive care, coordinating follow‑up and identifying health risks for patients in their homes. Providers schedule 20–40 hours per week with flexible options and no on‑call, weekends or holidays.

The role emphasizes evidence‑based practice, patient education, and seamless communication with primary care. Licensure and certification must be current or obtainable by start date.

Qualifications

• Active NP or PA license or ability to obtain by start date.
• Active national NP or PA certification or the ability to obtain national certification by start date.
• Ability to complete physical requirements including lifting a 30‑pound bag and navigating stairs.
• Reliable transportation to complete home visit assessments.

Responsibilities

• Conduct in‑home assessments including medical history review, medications reconciliation, vital signs and comprehensive physical exam.
• Perform screenings including point of care testing as appropriate.
• Identify diagnoses for care management and active medical management.
• Document assessment findings to inform members' PCP of gaps in care.
