Job summary

Housecall Providers Inc is seeking a Nurse Practitioner (NP) for primary care in the Portland metro area. The role focuses on chronic disease management, palliative care, and on-site as well as in-home visits, with a mix of urgent follow-ups as needed.

Responsibilities include comprehensive assessments, care coordination, and orders for labs or meds, while maintaining high-quality patient outcomes and productivity metrics.

Qualifications

• Master’s degree from an accredited advanced nurse practitioner program.
• Licensed nurse practitioner in the state of Oregon (ANCC or AANP certified).
• Drug Enforcement Administration (DEA) license.

Responsibilities

• Provide diagnosis, treatment, consultation, and follow-up care in collaboration with the patient’s PCP.
• Perform comprehensive health assessments including physical and psychosocial history.
• Make referrals to appropriate social or medical services as needed.
