Job summary

Housecall Providers in Portland, OR seeks an experienced Nurse Practitioner to deliver medical care and palliative services in patients' homes and via telehealth. The role focuses on chronic disease management, prevention, and end-of-life support, coordinating with a patient’s PCP to align care with goals.

This part-time NP position covers the Portland metro area, two days per week, with a competitive hiring range of $67.76 to $82.81 per hour.

Qualifications

• Master’s degree from an accredited advanced nurse practitioner program.
• Currently licensed as an NP in Oregon (ANCC or AANP).
• DEA license required; CPR certification at hire or within 6 months.

Responsibilities

• Provide diagnosis, treatment, and follow-up care in collaboration with the patient’s PCP.
• Perform comprehensive health assessments including physical and psychosocial history.
• Make referrals to social or medical services as needed.
• Address prevention of diseases and management of chronic conditions; support end-of-life care goals.
• Develop and implement proactive, patient-centered care plans with team input.
• Order tests, prescribe medications, and manage follow-up care.
• Coordinate with caregivers and facilities to ensure continuity of care.
• Educate patients and families on preventive care and treatment options.
• Participate in after-hours call schedule and quality improvement activities.
