Moms In Motion - A Virginia Service Facilitation Provider seeks a Service Facilitator (SF) Case Manager who will visit clients in their homes across Virginia. You’ll guide families through Medicaid Waiver programs, write care plans, and troubleshoot authorizations and timesheets.

Qualifications

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience matter more.
• Must pass a background check and provide 2 professional references.

Responsibilities

• Visit clients in their homes across VA.
• Serve as the go-to guide for waiver programs.
• Write care plans and assessments that make a difference.
• Troubleshoot service authorizations, timesheets, and related tasks.
• Build trusting relationships with families.
