Job summary

Alignment Health is seeking an Advanced Practice Clinician to join the Care Anywhere (CAW) program in the Fresno area. This field-based role supports patients at home, coordinating with PCPs and specialists to deliver proactive clinical care.

You will perform in-home assessments, medication reviews, and health screenings, with a focus on preventing hospitalizations and managing chronic conditions. A Master’s degree in NP or PA, current licensure, and EMR experience are required.

Qualifications

• Master’s degree in NP/PA program required.
• Active NP/PA license required with furnishing number.
• EMR experience preferred and strong clinical communication skills.

Responsibilities

• Conduct in-home assessments, medication review and health screenings.
• Provide patient education and align care plans with patient goals.
• Identify diagnoses for care management and medical management.
• Coordinate with PCPs, specialists, and ancillary providers.
• Collaborate with Regional Medical Officer and team on care plans.
• Lead clinical team such as nurses, health coaches, and care coordinators.
