Option Care Health, Inc. is the nation’s largest home infusion provider, offering nurses the opportunity to coordinate and deliver intermediate direct patient care in home or infusion suites.

The Infusion Nurse II role requires clinical leadership, adherence to physician plans of care, and collaboration with a multidisciplinary team to ensure safe, compliant patient care. Successful candidates will hold an active RN license, have at least one year of nursing experience, and maintain CPR

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit and documents outcomes for physicians.
• Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance.
• Teaches patients and families to achieve independence with prescribed therapy.
• Communicates with the clinical team, patients, and families to coordinate care and document activities.
• Coordinates discharge planning and prepares discharge summaries with instructions for patients.
