Job summary

Yale NewHaven Health is seeking a Home Infusion RN to deliver infusion therapy in patients' homes or ambulatory infusion settings. The role involves patient assessments, managing vascular access devices, and educating patients and caregivers to ensure safe treatment per physician orders.

The ideal candidate holds a Connecticut nursing license with at least three years IV-related experience, and preferably a BSN.

Qualifications

• RN licensure to practice in Connecticut
• Three years IV-related experience
• CRNI certification a plus
• Experience in home care or home infusion desired
• Strong customer service and communication skills

Responsibilities

• Provide nursing services at home and alternate sites for IV, subcutaneous, oral, etc. medications.
• Coordinate care with the physician, pharmacy team, and other members of the collaborative treatment team.
• Teach patients and/or family/caregivers about care needs, including self-administration when applicable.
• Maintain accurate and timely clinical nursing records according to policies.
• Adhere to infection control standards and regulations in home care nursing.
