Job summary

Yale New Haven Health seeks a Home Infusion Registered Nurse to deliver high-quality infusion therapy in patients' homes or ambulatory settings. You will assess patients, manage vascular access, educate patients and caregivers, and ensure safe administration according to physician plans of care.

Qualifications include an RN with CT licensure, a nursing diploma/ADN, and 3 years IV experience; BSN preferred and CRNI a plus. Weekend rotations may apply based on needs.

Qualifications

• Three years of IV-related experience.
• Experience in home infusion or home care is desirable.
• Case management experience is a plus.
• CT nursing licensure required or eligible.

Responsibilities

• Provide nursing services at home or alternate sites for infusion therapy.
• Coordinate care with physicians, pharmacy team, and other providers.
• Educate patients and caregivers on care needs and disease management.
• Maintain accurate nursing records and adhere to infection control standards.
• Ensure confidentiality and professional conduct; participate in staff activities.
