Job summary

Yale New Haven Health is seeking a Home Infusion RN to deliver high-quality infusion therapy and nursing services in patients' homes or alternate settings. You will assess patients, manage vascular access devices, educate families, and document care per physician plans.

The role requires 3 years of IV experience with Connecticut licensure, and a nursing diploma or higher. CRNI is a plus; home infusion knowledge is preferred.

Qualifications

• Nursing diploma or associate degree in nursing. Bachelor's degree in nursing preferred. CRNI certification a plus.
• Three years of IV related experience.
• Previous home care or home infusion experience desirable.
• Case management experience is a plus.

Responsibilities

• Provides nursing services at home and/or alternate sites for intravenous, subcutaneous, oral, etc. medication administration for YNHHS Home Infusion Pharmacy patients in compliance with YNHHS policies.
• Collaborates with physicians, pharmacy team, and care colleagues to coordinate patient care.
• Educates patients and caregivers on care needs, self-administration when applicable, and disease prevention.
• Maintains accurate and timely nursing records per policies.
• Adheres to ethical nursing standards and patient confidentiality.
• Performs other duties as assigned by home infusion leadership and represents the department professionally.
• Keeps current with infusion therapy developments and complies with policies and standards.
• Participates in staff meetings and committees as requested by the Manager of Home Infusion.
• Follows infection control measures for home care and IV nursing services.
• Contributes to development and implementation of practice standards and regulatory compliance.
