InfuCare Rx is seeking a Pharmacy Service Representative to advocate for patients and ensure prescribed drug therapy is followed and aligned with care plans in the home infusion setting.

Reporting to Pharmacist-In-Charge, you will coordinate with staff pharmacists to support medication delivery, patient assessments, and education on digital platforms, while maintaining accurate records and compliance.

Competenze

• Healthcare experience required.
• Pharmacy Technician license in the state of employment.
• Specialty Pharmacy and home infusion experience preferred.
• Proficiency with Microsoft Office suite.

Mansioni

• Communicates with patients to ensure proper delivery of medications (acute, IVIG and specialty) for scheduled infusion appointments.
• Understand how to properly complete assessments during patient communications.
• Assist in education for the patient on using the Digital platform.
• Ensure patients receive notifications and understand appointment schedules.
• Respond to patient calls in a timely and appropriate manner.
• Update patient demographic and related data according to policies and procedures.
