Job summary

Amerita, an affiliate of BrightSpring Health Services, seeks a Pharmacist to provide pharmaceutical care for customers/patients in the service area, monitor outcomes, and coordinate dispensing of medications and related supplies. Collaboration with physicians, nurses, and other healthcare professionals is essential, with on-call duties as required.

Shift: Monday-Friday 8:30am-5:30pm. Requires Bachelor of Science in Pharmacy; Doctor of Pharmacy preferred.

Qualifications

• Bachelor of Science in Pharmacy is required; Doctor of Pharmacy preferred.
• Minimum of two years of recent experience in an acute-care setting or one year in home infusion therapy.
• Experience with compounding infused drugs is preferred.

Responsibilities

• Evaluates and monitors drug therapy relative to the total needs of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
• Adheres to organization policies and procedures and all applicable laws and regulations governing pharmacy practice.
• Coordinates and facilitates new referrals and admissions through collaboration with nurses, customer service and distribution.
• Reviews, receives and verifies all new patient orders and documents on the respective order form.
• Maintains thorough and accurate documentation in patient records of clinical activities and communications.
