Job summary

Amerita, Inc. is seeking a Pharmacist to join our home infusion pharmacy team. You will evaluate and monitor drug therapy for home care patients, collaborate with physicians, nurses and the distribution team, and ensure compliance with state and federal regulations.

Qualifications

• Bachelor of Science in Pharmacy required; Doctor of Pharmacy preferred.
• Registered Pharmacist license in state(s) of practice.
• Experience in acute-care or home infusion therapy preferred.

Responsibilities

• Evaluates and monitors drug therapy for home care patients in collaboration with physicians, nurses and other healthcare professionals.
• Ensures compliance with state and federal laws, regulations and USP standards governing pharmacy practice.
• Coordinates referrals and admissions with nurses, customer service and distribution.
• Reviews and documents new patient orders and supports patient care planning at home.
• Oversees pharmacy technician functions as required by Pharmacy Board rules.
