Job summary

Option Care Health is seeking a Registered Nurse to provide on-site and home-based infusion support, coordinating referrals, patient assessments, education, and training. You will work with pharmacists, IP teams, discharge planners, and home health agencies to ensure safe initiation and ongoing management of home IV therapy.

Responsibilities include initial assessments, developing treatment plans, and documenting education and progress, while maintaining compliance with policy and state nursing

Qualifications

• Active RN license in the United States, currently in SC per posting.
• Minimum 2 years' experience in home IV therapy and patient education.
• Excellent clinical, communication and organizational skills.

Responsibilities

• Perform initial patient assessment to determine home IV therapy suitability.
• Coordinate initiation of home IV therapy with pharmacy, operations and reimbursement teams.
• Educate patient and caregivers on IV therapy administration and devices.
• Train home health staff when needed and document training outcomes.
• Document all assessments, plans, and monitoring for care coordination.
