Job summary

Option Care Health is seeking an experienced RN to provide on-site home IV therapy coordination and patient education in South Carolina. The role includes patient assessment, training, and collaboration with pharmacy, clinical and reimbursement teams to ensure safe home infusion services.

Responsibilities include coordinating care plans, communicating with referring agents, and ensuring compliant documentation. Excellent clinical and organizational skills are essential for success in this role.

Qualifications

• RN license issued in South Carolina.
• Current South Carolina driver\'s license.
• 2+ years experience in home IV therapies and patient/family education/training skills.
• Excellent clinical, communication and organizational skills.

Responsibilities

• Performs an initial patient assessment to determine home IV therapy candidacy.
• Determines patient eligibility against acceptance criteria.
• Documents assessments completely and timely.
• Collects information to assess reimbursement and clinical needs.
• Develops initial treatment plan with pharmacist and IP team for home infusion therapy initiation.
• Educates patient or caregiver on home IV therapy per policy and protocols.
