Job summary

Option Care Health, Inc. is seeking a registered nurse to provide on-site home IV therapy services, patient assessment, education, and coordination with pharmacists, IP teams, and discharge planners.

The role involves nurse-led training, documentation, and collaboration to ensure safe home infusion care for patients. Required are SC RN licensure, current driver's license, and 2¾ years of relevant experience, with strong clinical and communication skills.

Qualifications

• RN license in good standing (SC)
• Current South Carolina Driver's License
• 2¾ years of experience, preferably in home IV therapies and patient/family education/training
• Excellent clinical, communication and organizational skills

Responsibilities

• Performs an initial patient assessment to determine home IV therapy eligibility
• Determines if patient meets acceptance criteria for Intramed Plus
• Documents patient assessment completely and timely
• Obtains clinical information to assess reimbursement and clinical needs
• Develops initial treatment plan with the pharmacist and IP team for initiation of home infusion therapy
• Communicates assessment back to referring agent and IP team
• Educates patient or caregiver on home IV therapy per policy and protocols
• Trains patient or caregiver on administration techniques and device operation
• Certifies patient/caregiver competency for home IV therapy when applicable
• Documents education/training process promptly
• Educates home health nursing agency personnel as needed
• Coordinates initiation of home IV therapy with Pharmacy, Operations and Reimbursement
• Maintains a professional relationship with case managers and physicians at assigned accounts
• Works with hospital discharge managers to facilitate timely safe discharge
• Completes admission and revisits documentation timely
• Handles On-Call responsibilities professionally
• Documents On-Call activities per policy
• Monitors patient progress during hospitalization and home therapy
• Identifies and triages problems and provides recommendations
• Communicates care plan revisions to the branch as needed
• Maintains updated treatment plan and documentation per protocol
