Option Care Health is seeking a registered nurse to coordinate, assess and educate patients for home IV therapy. The role includes on-site referral coordination, patient education, and collaboration with pharmacists and cross-functional teams to ensure safe home infusion care.

Responsibilities include patient assessment, training patients or caregivers, and coordinating with discharge planners and physicians to optimize care plans.

Qualifications

• RN license current in SC? (for example)
• 2+ years experience in home IV therapies or education/training skills
• Excellent clinical, communication and organizational skills

Responsibilities

• Performs initial patient assessment for home IV candidacy.
• Develops initial treatment plan with pharmacist and IP team.
• Educates and trains patient or caregiver on home IV therapy.
• Coordinates with physicians, nurses and case managers.
