Job summary

Option Care Health is the nation’s largest home infusion provider, delivering comprehensive home IV therapy services across the United States. This role focuses on on-site referral coordination, patient assessment, and education to ensure safe, effective home infusion care.

The RN will assess candidates for home IV therapy, coordinate with pharmacists and interdisciplinary teams, train patients and caregivers, and support discharge planning.

Qualifications

• RN license required and current in South Carolina.
• Current SC Driver's License required.
• 2+ years experience, preferably in home IV therapies, patient/family education/training.
• Excellent clinical, communication and organizational skills.

Responsibilities

• Performs initial patient assessment to determine eligibility for home IV therapy.
• Develops initial treatment plan with pharmacist and IP team.
• Educates patient or caregiver on home IV therapy according to policy.
• Trains patient or caregiver on administration techniques and device operation.
• Coordinates with discharge planners, case managers, and home health agencies for therapy initiation.
• Documents coordination activities and patient progress in a timely manner.
• Maintains professional relationships with physicians and other care team members.
• Triages problems and provides recommendations to resolve issues.
