Job summary

Naven Health, Inc. in Chattanooga, TN seeks a Special Programs Nurse to coordinate and provide skilled nursing care in the home or alternate infusion sites, ensuring patient safety and continuity of care per physician orders.

Requires an active RN license, 5+ years nursing, 3 years in specialty therapies, and current BLS certification. Travel 100% of the time to patient homes and sites is expected.

Qualifications

• Graduate of an accredited School of Nursing.
• Active and unrestricted registered nurse license.
• Minimum of 5 years nursing experience.
• Minimum of 3 years of experience in specialty therapies.
• Current Basic Life Support certification for Health Care Professionals.

Responsibilities

• Plan, provide, supervise and document professional nursing care per physician orders and policies.
• Provide direct patient care: perform treatments, administer medications, educate patients and families.
• Assess patient needs and re-evaluate care through visits and data review.
• Communicate findings to the clinical team and document in real time for billing.
• Coordinate discharge planning and prepare summaries with patient instructions.
