Job summary

Naven Health is a nationwide home infusion nursing network focused on delivering specialized infusion care. The Special Programs Nurse coordinates and provides skilled nursing care in the home or alternate infusion sites, ensuring patient safety while aligning with physician plans of care.

Under supervision, the nurse delivers direct patient care, documents activities in real-time, and interacts with the clinical team to ensure continuity of care and timely billing documentation.

Qualifications

• Graduate of an accredited School of Nursing.
• Active and unrestricted registered nurse license.
• Minimum 5 years nursing experience with infusion therapies preferred.

Responsibilities

• Plan, provide, supervise and document professional nursing care in the home or alternate sites.
• Administer medications and educate patients and families.
• Assess patient needs at visits and adjust plans of care as needed.
• Coordinate discharge planning and communicate with clinical team.
• Maintain accurate real-time nursing documentation for billing and compliance.
