Job summary

Soleo Health is seeking a Per Diem Infusion Nurse to support infusion patients in the Saginaw, MI area. You will provide direct patient care, education, and care coordination in home or alternate sites, adhering to care plans and regulatory requirements.

Must live within 25 minutes of Saginaw, MI and be willing to travel up to 2 hours one way. As a registered nurse with infusion experience, you’ll work a flexible schedule (20-24 hours/week) including days/evenings and on-call rotations, driving

Qualifications

• Current RN license by the state’s Board of Nursing, in good standing
• RN degree, BSN preferred
• Advanced certification in specialty areas (CRNI/ONS/IgRN) preferred but not required
• Administrative Oversight: manage workflows and resources
• Emergency Availability: nights, weekends, holidays as required
• Minimum 3 years nursing infusion experience preferred

Responsibilities

• Provide direct care for assigned patients including communication with care team
• Educate patient/care partner to safely administer home infusion therapy
• Assess home environment and ensure safety for prescribed care
• Develop and evaluate plan of care with team
• Manage schedule and communicate changes to team
• Coordinate patient care via approved platform
• Document patient encounters per policy and regulatory requirements
• Maintain knowledge of Soleo therapies and standards
• Submit POTs and clinical documents per policy
