Job summary

Option Care Health is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in home or infusion suites, under a physician’s plan of care. This role requires an active RN license and CPR certification, with at least 1 year of nursing experience.

Strong documentation, communication, and teaching skills are essential. The position involves coordinating with clinical teams, education for families, and adherence to HIPAA and regulatory standards.

Qualifications

• Active and unrestricted RN license in state of practice.
• Minimum of 1 year of nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing care plans; administers treatments and medications.
• Assesses patient needs at each skilled visit and communicates significant changes to the physician.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Communicates with clinical teams, patients, and families to coordinate and document care.
• Maintains HIPAA-compliant documentation and supports discharge planning as needed.
