Job summary

Option Care Health, Inc. is seeking an Infusion Nurse II to coordinate and provide intermediate direct patient care in home or infusion settings under a physician’s plan. The role requires nursing licensure, state practice, and a structured supervision model.

The ideal candidate will have 1+ year of nursing experience, CPR certification, and a commitment to compliant, patient-centered care while working with the clinical team to optimize care delivery and documentation.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 year nursing experience.
• Current CPR certification required.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit; reevaluates needs and informs physician of significant changes.
• Provides training and mentorship to Infusion Nurse I’s to ensure patient safety and compliance.
• Provides effective teaching to patients and families to achieve independence with prescribed therapy.
• Communicates with Clinical team, medical providers, patients, and families to facilitate continuity of care and completes documentation in real time.
