Job summary

Option Care Health, the nation’s largest home infusion provider, seeks an Infusion Nurse II to coordinate and deliver intermediate nursing care in home or infusion suites. You will work under supervision to ensure patient safety and continuity of care under a physician plan of care.

Responsibilities include developing care plans, educating patients and families, and documenting care in real-time. Requires active RN license, 1+ year nursing experience, and CPR certification.

Qualifications

• Active and unrestricted RN license in the state of practice.
• Minimum of 1 years of nursing experience.
• Current CPR certification required.
• Demonstrated competency in patient care standards.

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit; re-evaluates patient needs and informs physician of changes.
• Provides training and mentorship to Infusion Nurse I’s as needed to ensure patient safety and compliance.
• Provides effective teaching to patients and families for independence with prescribed therapy and care needs.
