Job summary

The University of Vermont Health Network in Burlington, VT seeks a Registered Nurse - Home Infusion Coordinator to ensure safe transition of patients to home infusion, create and assess the plan of care, and teach patients, caregivers and home health nurses to administer infusion therapy at home.

The HINC coordinates with the health care team to ensure timely progression across the continuum of care, maintains contact with patients by phone weekly or as needed, supports quality improvement, and

Qualifications

• Current RN licensure in Vermont or compact licensure recognized by the State of Vermont.
• ADN required, BSN preferred.

Responsibilities

• Coordinate patient plans of care for home infusion.
• Educate patients, caregivers and home health nurses to administer infusion therapy at home.
• Coordinate with health care team to assure timely progression across continuum of care.
• Maintain contact with patient and care providers through weekly phone assessments.
• Support quality improvement through evaluation of variances to protocols and maintain quality data systems.
