Job summary

Yale NewHaven Health is seeking an Home Infusion Nursing Coordinator to lead the Home Infusion Nursing Team in coordinating patient care, education, and staff training for infusion therapies at the home setting. The position includes direct patient visits as needed and partnership with pharmacy and nursing leadership to ensure high-quality care.

The role focuses on standards of practice, regulatory compliance, and development of competency-based education for staff in a fast-paced infusion

Qualifications

• RN, Bachelor's degree from an accredited college or school of nursing.
• 3–5 years IV related experience; 3 years of home health care experience; 2 years of Case Management experience.
• Current licensure as a Registered Nurse in Connecticut.
• Must have excellent customer service skills and effective interpersonal skills; proficient computer skills.

Responsibilities

• Develops, maintains and grows admissions to home infusion through relationships and provides responsive customer service.
• Determines referral eligibility by payor source, safety risk and patient needs.
• Collaborates with referral sources and infusion staff to coordinate care for new patient referrals.
• Coordinates and communicates home infusion activities; provides direct patient visits and education on infusion handling.
• Leads standards of practice development; ensures regulatory compliance in nursing.
• Reviews and responds to patient/family complaints; investigates events and follows up.
• Represents nursing in interprofessional learning activities and education.
• Supports development of nursing leadership; mentors staff through education and certification.
• Attends meetings, trainings and regional conventions as directed.
• Coordinates cross-training of departmental duties to maximize productivity.
• Keeps abreast of infusion therapy developments and new care models.
• Complies with company policies and directives.
• Communicates problems to Pharmacy and Nursing leadership.
• Assesses learning needs per regulatory requirements and evidence-based practice.
• Considers different nursing experience levels when supporting staff.
• Acts as education and practice expert; represents home infusion pharmacy.
• Partners with colleagues across specialties to ensure quality content.
• Uses technology to improve education programs.
• Supports ANA Standards of Practice in nursing education.
• Mentors staff to develop professionally through education and certifications.
• Acts as a role model to promote the nursing profession.
• Identifies learning opportunities from quality metrics.
• Gathers and communicates best evidence to support bedside practice.
• Uses specialty nursing standards to keep programs current.
• Develops, implements and evaluates competency-based orientation.
• Manages ongoing education and competency development across staff.
• Identifies learning needs with leadership and regulatory collaboration.
• Develops continuing education programs with partners.
• Provides ongoing education to support competency growth.
