Job summary

The position requires an LPN/RN license (CA) with at least three years of nursing experience, plus strong organizational, communication, and HIPAA-compliant practices to ensure safe, timely admissions to NuFactor services.

Qualifications

• High school diploma or equivalent.
• Completion of an accredited practical nursing or registered nursing program.
• Full, current, and unrestricted LPN or RN license in the state of residence.
• IgCN certification within two years for RNs and ongoing compliance.
• Minimum of three nursing years of experience.

Responsibilities

• Serve as central clinical resource to internal/external staff; provide after-hours/weekend nursing support as needed.
• Collaborate with physicians, staff, patients, families to obtain medical documentation supporting treatment coordination and reimbursement.
• Facilitate safe transition of patients to home care; evaluate placement needs and home environment suitability.
• Support development and management of subcontracted nursing services and independent contractors.
• Review nursing documentation for therapy appropriateness, outcomes, and compliance with standards.
• Contribute to policy/procedure development and departmental initiatives.
• Maintain knowledge of industry trends and pursue continuing education.
