Job summary

NuFactor seeks a Clinical Nurse Coordinator to collaborate with Admissions, Pharmacy, and Client Service Specialists to support the safe, timely admission of patients to NuFactor services. You will coordinate nursing schedules for an assigned caseload to promote continuity of care and timely service delivery.

The role serves as a clinical and operational resource to subcontracted nursing agencies and internal staff, including after-hours telephone support and cross-functional coordination across

Qualifications

• High school diploma or equivalent is required.
• Completion of an accredited practical nursing or registered nursing program.

Responsibilities

• Coordinate patient care activities and nursing schedules for an assigned caseload.
• Serve as a central clinical resource regarding nursing matters, including after-hours support.
• Collaborate with physicians, clinical staff, patients, and families to obtain medical documentation.
• Facilitate safe transition of patients to home care services and evaluate placement needs.
• Support development and management of subcontracted nursing services and resources.
• Review nursing documentation for therapy appropriateness and outcomes.
