Job summary

Capstone Vital Care is seeking an Intake Coordinator to support the referral-to-start-of-care process for acute and specialty home infusion patients. You will verify benefits, gather documentation, estimate patient responsibility, and coordinate with patients, providers, insurers, and internal teams.

The role emphasizes accuracy, timely access to care, and HIPAA-compliant handling of sensitive information within a collaborative, patient-centered environment.

Qualifications

• High school diploma or equivalent.
• Experience with insurance eligibility and benefits verification.
• Ability to manage multiple referrals and prioritize tasks.

Responsibilities

• Receive and process new patient referrals for acute and specialty home infusion therapies.
• Enter patient demographics, insurance information, and referral details into the pharmacy management system.
• Review prescriptions, clinical records, and payer information for completeness and accuracy.
• Coordinate with referral sources, patients, and internal teams to obtain missing documentation.
• Perform insurance eligibility and benefit verification for medications, equipment, and services.
• Communicate benefit information and estimated financial responsibility to patients and caregivers.
• Collaborate with Authorization Department on prior authorizations and documentation requirements.
• Support billing and collection efforts with accurate insurance and patient information.
• Protect patient confidentiality and HIPAA compliance.
