Job summary

Talentify is seeking an Intake Coordinator to serve as the liaison between referral sources and the Pharmacy, ensuring a smooth transition from acute care to home infusion for patients.

You will establish relationships with referral sources, procure patient information for admission decisions, verify insurance, obtain authorizations, and coordinate billing and discharge planning. A strong background in medical terminology and insurance processes is essential.

Qualifications

• High school diploma or equivalent required.
• 2-5 years of medical terminology, insurance verification, and/or medical services experience required.
• 1-2 years of experience with home infusion preferred.
• 2-3 years experience with home care, discharge planning, or insurance billing a plus.
• Maintain a current insurable driver's license.
• College degree is desirable.

Responsibilities

• Establishes and maintains relationships with referral sources by receiving and processing intake information for potential patients who require home infusion therapy.
• Facilitates intake process by procuring all patient information necessary to determine home infusion admission based on clinical data and policies.
• Verifies health insurance coverage using verification forms.
• Obtains/maintains authorization numbers and coordinates case management updates.
• Prepares and manages paperwork for billing and collections and communicates acceptance or denial to referral sources.
• Coordinates discharge planning with referral sources, nursing agencies, pharmacy, and patient/caregiver.
