Option Care Health is the nation's largest home and alternate site infusion services provider seeking an experienced Clinical Transition Specialist. You will educate patients and staff, drive referrals, and ensure proper placement of patients in home health care settings.
The role requires an RN or Pharmacist license with at least 2 years in healthcare, strong relationship skills, and travel to care facilities. Competitive benefits and pay range are provided.

- Licensed RN or Pharmacist with state license.
- Minimum 2 years in healthcare experience.
- Experience building relationships with internal/external partners.
Mansioni
- Maintain and grow relationships with referrals to drive sales and patient starts.
- Serve as a primary contact for referral sources, educating about services.
- Educate patients, family and facility staff about home/infusion services.
- Assess patient suitability for alternate site care and coordinate with hospital staff and case managers.
- Coordinate care transitions to home or ATS with internal partners to ensure smooth discharge.
