Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license in the State Board of Pharmacy.
- One of the following: sterile IV admixture experience, non-sterile compounding/immunizations/MTM, ASHP residency, APPE, or equivalent.
Responsibilities
- Determines suitability of patients for home care and collects necessary data.
- Designs, monitors and evaluates patient-specific pharmacy care plans for home care.
- Provides medication-use education to staff, patients and caregivers.
- Ensures continuity of pharmaceutical care and accurate dispensing per policies.
