Entry-level clinician position with opportunities to grow in a nationwide home infusion setting. Requires an active state license and completion of accredited pharmacy education.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• Preferred: ASHP accredited Residency or equivalent experience.

Responsibilities

• Determines the suitability of patients for home care and collects necessary information.
• Designs, monitors, and evaluates patient-specific pharmacotherapy plans for home care.
• Provides medication education to patients, caregivers, and staff.
• Ensures safe dispensing, labeling, and compliance with laws and quality standards.
