Job summary
Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe and appropriate pharmacy services for home care patients. You will collaborate with Nursing, educate patients and caregivers, and ensure adherence to regulatory standards.

Qualifications
- Graduate of an accredited School of Pharmacy.
- Current, active Pharmacy license as granted by the State Board of Pharmacy.
- One of the following: hospital/home care experience, MTM, residency or equivalent.
Responsibilities
- Determine suitability of patients for home care and collect necessary information.
- Design and monitor patient-specific pharmacy care plans for home care patients.
- Provide medication-use education to staff, patients and caregivers.
- Ensure compliance with state/federal laws and organization policies.
