Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to deliver safe pharmacy services in home care settings. You will coordinate with nursing staff, monitor progress toward goals, and educate patients and caregivers on medication use.

The role requires a Pharmacy degree, a valid license, and familiarity with IV admixture or related areas. You will support continuity of care and adhere to regulatory standards while growing professionally in a leading home infusion organization.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license by state board.
• Ability to work in sterile IV admixture or related settings.
• Familiarity with pharmacy regulations and safety practices.

Responsibilities

• Assess patient suitability for home care and collect required data.
• Design and monitor patient-specific pharmacy care plans.
• Modify plans based on monitoring data and lab values.
• Educate patients, caregivers and staff on medication use.
• Ensure continuity of care across home and other settings.
