Job summary

Option Care Health, Inc. is seeking a Clinical Pharmacist to provide safe and appropriate pharmacy services to patients in the home infusion setting. This entry-level position coordinates with the Nursing Department and educates patients and caregivers on medication use.

The role requires a PharmD degree, an active State license, and a willingness to travel as needed. You will support medication therapy management and participate in quality and safety initiatives.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of multiple pathways showing pharmacy practice experience or residency

Responsibilities

• Determine suitability of patients for home care and collect necessary information.
• Design and monitor patient-specific pharmacy care plans for home care.
• Provide medication-use education to staff, patients and caregivers.
• Ensure continuity of pharmaceutical care across home and other settings.
• Manage medication administration equipment and report discrepancies.
• Respond to drug information requests and participate in CEU opportunities.
• Comply with all regulatory, safety and quality requirements.
