Job summary

Option Care Health, the nation’s largest home infusion provider, seeks an entry-level Clinical Pharmacist to deliver safe pharmacy services in the home care setting. You will coordinate with Nursing, monitor progress toward goals, and educate patients, caregivers and staff on medication use.

Responsibilities include designing care plans, evaluating therapy, and ensuring proper dispensing and safety practices.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• Experience providing customer service to internal and external customers.

Responsibilities

• Determines the suitability of individual patients for home care and collects necessary information.
• Designs, monitors and evaluates patient-specific pharmacy care plans for home care.
• Provides medication-use education to staff, patients and caregivers.
• Ensures continuity of pharmaceutical care between home and other patient-care settings.
