Option Care Health in Ashland, VA, is hiring a Clinical Pharmacist in a full-time capacity. The role focuses on safe and appropriate pharmacy services for home care patients, coordinating with nursing staff, educating patients and caregivers, and ensuring continuity of pharmaceutical care across care settings.

The position requires pharmacy licensure in the state, a Doctor of Pharmacy degree, and experience or training in sterile IV admixture, MTM, or equivalent. Travel up to 5% may be required.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• One of the following: at least 1 year of experience in sterile IV admixture procedures or equivalent.
• ASHP residency or advanced pharmacy practice experience preferred.

Responsibilities

• Determines suitability of home care patients and collects necessary information.
• Designs and monitors patient-specific pharmacotherapy plans for home care.
• Provides medication-use education to staff, patients and caregivers.
• Ensures safety, quality, and regulatory compliance in all dispensing and care activities.
