Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe, effective pharmacy services for home care patients. You will coordinate with Nursing and other teams, monitor patient progress, and educate patients, caregivers and staff on medication use.

The role emphasizes pharmacokinetic assessment, lab value interpretation, and adherence to regulatory standards in an alternate-site setting. A pharmacy license and PharmD are required.

Qualifications

• Graduated from an accredited School of Pharmacy.
• Current, active Pharmacy license issued by the State Board.
• Experience in sterile IV admixture, MTM, or ASHP residency/advanced practice is preferred.

Responsibilities

• Determines patient suitability for home care and collects necessary information.
• Designs and monitors patient-specific pharmacotherapy plans for home care.
• Provides medication-use education to patients, caregivers and staff.
